Just a quick reminder.

We have noticed that many of the clubs and schools are advertising for their musters and trials using Google Forms or Microsoft Forms.  

Whilst we understand there are reasons behind this, we would like to remind you that all the individuals (players, coaches, managers, umpires, other volunteers) must also complete the PPTANC Registrations form.  

Each club and school have their own form where you can add customized fields to capture the information that you are wanting on Google/Microsoft Forms.  If you use this form, it will save the individuals filling out 2 forms.

For those clubs that wish to use Google/Microsoft forms, please note - it is the individual (or parent or legal guardian if under 16) that is required to fill in the PPTANC Registration Form.  By submitting the form, the respondent agrees to abide by the PPTANC Code of conduct, along with the other Terms and Conditions stated on the form.  

To ensure that each person knows what they are agreeing to, can you please ensure that the forms are completed by the individuals and not by the club/school on behalf of the individual.
